From the reading I have done I concluded that the software used to track risk was compromised in favor of the greedy. Other articles online talked about how they left out factors or changed parameters so the risk was never truly shown. When you own the system that tracks risk and is not regulated or evaluated there will be those who choose to put greed ahead of prudence.
