Not all of the health insurance plans analyzed by the National Committee for Quality Assurance for America's Best Health Insurance Plans made their data available. Of 730 plans, 120 either declined to provide data to NCQA or reported it but would not permit it to be made public. The plans listed here are the largest nonreporters.
